MEDICATION MANAGEMENT

 If you are seeking medication management, have your primary care provider, or another specialist that has treated you for your pain, fax a referral to our office at 918-380-9838.

It is important that we have a note from the previous provider regarding your pain problem and any pertinent imaging reports that may be available (such as MRI, CT, X-RAY). If it is not included in the referral, we will contact you and the provider's office to get additional information. 

Once we have the medical records, we will verify your insurance and determine eligibility for services. Please see our list of accepted insurance. If we do not accept your insurance or you are a self pay patient, you will be notified of the fees before your appointment. 

Once your referral is received and reviewed along with the insurance, you will be contacted via phone call to schedule an appointment with one of our providers. 

You can expect to be in the office between 1 to 2 hours for your initial appointment. 

All the initial paperwork is completed in the office. Please show up approximately 30 minutes early to complete your paperwork.
